Um, I think both _unhealthiness through obesity_ is the problem that's attempting to be solved; it just happens to occur most frequently in... well... fat people.

Call me crazy, but I don't think it's a stretch to fund community health efforts by using problems within community health that are easiest to quantify. Obesity is one of those things that's easiest to check when it's solved.
