Sometimes "bless your heart" isn't a direct insult, you're right TL. Often it's conveying fake sympathy. "Oh you have a cold, glad it's not me!" and "Don't sneeze on me" is what they really mean. I rarely hear a genuine blessing spoken as "bless your heart."
There are plenty of other comments in the same category. "Oh, that's a good school" is one I hear frequently when someone talks about a school from another region to someone who rarely, if ever, leaves the South. Reading the body language and intonation make it clear that there is no recognition of the school.
